  • All X-Rays, CAT scans, MRIs *this includes the disk or films and the written radiologists report*
  • Lab Results
  • Medical Records
  • Workman's comp. information
  • Name and address of family physician
  • List of current medications and allergies
  • List of previous surgeries and medical problems
  • A parent or legal guardian must be present with all patients under the age of 18

    We will file your claim for you to most insurance companies. We are contracted with Anthem, Rocky Mountain Health Plans, Cofinity, PacifiCare, TRPN, NPPN, Plan Vista, Pinnacol Assurance, Tricare, Medicare and Colorado Medicaid.
